This form is required in order to be reimbursed for medical travel.

These medical transportation benefits apply to you if you are traveling to:

  • Medical services covered by MSP or a public health agency

  • Traditional healers if approved as an exception

  • Services under the Health Benefits Program (ie: dental, vision)

Exceptions are no longer required for travel to substance use treatment programs that are licensed under the Community Care and Assisted Living Act or registered through the Assisted Living Regulation. This includes facilities listed under:

  • Licensed Treatment and Supportive Recovery Services

  • Registered Recovery Services

This change is intended to streamline administrative processes for those seeking access to treatment, particularly in time-sensitive situations. Information on how to apply for funding toward the cost of attending a non-FNHA-funded treatment centre is available at fnha.ca

As of December 1, 2025, FNHA updated meal rates, the commercial accommodation rate guide, and removed certain Exception requirements under the Medical Transportation (MT) Benefit. To view all Medical Transportation Benefit Updates, view the PDF by clicking the icon below.
